Respiratory muscle strength in stroke patients affects 6-minute walk test results

Stroke patients will soon have respiratory problems in the acute and subacute phases, such as sleep disorders, changes in ventilation patterns. However, the evaluation and treatment of respiratory muscles are rarely involved in neurological rehabilitation programs for stroke patients.

There are many ways to measure respiratory muscle strength. The most commonly used indicators are maximum expiratory pressure (MEP) and maximum inspiratory pressure (MIP). The MEP decline represents a decline in the tester's torso control ability, while a decrease in MIP is associated with a slower walking speed. However, few of these methods have been used to evaluate the function of patients with chronic stroke.

In order to compare the differences in respiratory muscle strength between healthy and stroke patients of the same age and gender, and to investigate the correlation between respiratory muscle strength and the walking distance of the 6-minute walk test, Dr. Luz from the Department of Physiotherapy, University of A Coruña, Spain and his research The team conducted relevant research and published it in the Archives of Physical Medicine and Rehabilitation in January 2016.

This test is a cross-sectional study. After the selection, a total of 30 chronic stroke patients with hemiplegia or hemiparesis but able to walk were included in the trial, and 30 healthy and gender-matched healthy people were invited.

The maximum inspiratory pressure (MIP) and the maximum expiratory pressure (MEP) were measured as a measure of respiratory muscle strength by a pressure sensor and an expansion nozzle. Each indicator is measured three times and takes the maximum value. After recording the basic vital signs, the walking distance and walking speed of the 6-minute walking test of the two groups were measured. In addition, the testers also evaluated the exercise index and the stroke impact scale for stroke patients.

The investigators found that the MIP and MEP values ​​of stroke patients were significantly lower than those of healthy controls. Moreover, these data are also clinically significant because the MIP and MEP values ​​of stroke patients are less than 60% of the predicted value. When using the Pearson coefficient test, it was found that the 6-minute walk test was significantly positively correlated with MIP.

Therefore, the researchers believe that in the future, rehabilitation physicians and therapists should consider the respiratory muscle weakness in the development of rehabilitation treatment for stroke patients, and give necessary assessment and treatment to improve the treatment effect.
